The 'Send me a copy' option will be turned on by default when you add any of our form templates or create your own forms.
If you do not want to include the 'Send me a copy' option on your form please follow these steps to turn it off:
Log in to your Gym Forms Account > Click on 'Form Manager' > Click on 'Edit Form' > Select the form from the dropdown list > Click on 'Form Settings' > Click on 'Form Submission' > Untick 'Add 'Send Copy' option to form?' > Click on 'Save'.
